This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Grand Parkway Dental Studios is committed to protecting the privacy of your protected health information ("PHI"). This notice explains how we may use and disclose your health information and describes your rights regarding that information.

How we may use and disclose your health information

We may use and disclose your PHI, without your separate authorization, for the following purposes:

• Treatment — to provide, coordinate, and manage your dental care, including sharing information with other providers involved in your treatment.
• Payment — to bill and collect payment for the care we provide, including verifying coverage and processing claims.
• Health care operations — to run our practice, such as quality review, staff training, and scheduling.
• Appointment reminders and options — to contact you with reminders or to tell you about treatment alternatives and services that may benefit you.

Other uses and disclosures permitted or required by law

We may use or disclose your PHI when required or permitted by law, including for public health activities, health oversight, judicial and law enforcement purposes, to avert a serious threat to health or safety, for workers' compensation, and to comply with other legal obligations.

Uses and disclosures that require your written authorization

Most uses and disclosures of psychotherapy notes, uses and disclosures for marketing, and any sale of your PHI require your written authorization. You may revoke an authorization in writing at any time, and we will stop using or disclosing your information for that purpose going forward.

Your rights regarding your health information

• Access — you may inspect and request a copy of your health and billing records.
• Amendment — you may ask us to correct information you believe is incomplete or inaccurate.
• Accounting of disclosures — you may request a list of certain disclosures we have made.
• Restrictions — you may request limits on how we use or share your information, and we will accommodate reasonable requests where required.
• Confidential communications — you may ask us to contact you a certain way or at a certain location.
• Paper copy — you may request a paper copy of this notice at any time.
• Breach notification — you have the right to be notified if a breach of your unsecured PHI occurs.

Our responsibilities

We are required by law to maintain the privacy of your PHI, to provide you with this notice of our legal duties and privacy practices, and to abide by the terms of the notice currently in effect.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to this notice

We reserve the right to change this notice and to make the revised notice effective for information we already have as well as information we receive in the future. The current notice will always be posted on this page.
